A Case of Infantile Lobar Emphysema with Congenital Heart Disease

Abstract
Infantile lobar emphysema is a rare disease and a cause of respiratory distress during infancy. It is characterized by over-expansion of the affected lobe of the lung, tends to have a progressive course and shows repeated episodes of pulmonary infection. The common locations of pulmonary involvement are the left upper lobe, right middle and upper lobe. Once suspected, the diagnosis can be confirmed by a plain chest X-ray. Appropriate surgical therapy is usually necessary. We experienced a case of infantile lobar emphysema with congenital heart disease(double outlet of right ventricle with interruption of aortic arch) in a 7 month old male infant. Chief complaints were repeated dyspnea and tachypnea. Correct diagnosis was delayed because the chest radiograph was dominated by pneumonia. Chest X-ray and CT revealed a hyperinflated right middle lobe and the mediastinal shift to left side. This patient was successfully treated with right middle lobectomy. We report this case with a brief review of related literatures.
